EXPEDITED CHILD'S PASSPORT INSTRUCTIONS:
You are eligible to apply for a new passport if one of the following is true:
- You are eligible to apply if the applicant is under age 16 and the required parent(s) appear in person, or if one parent cannot appear and submits the consent form (Form DS-3053).
- If one of the parents cannot appear in person, the parent must complete the consent form (Form DS-3053) and provide a copy of front and back of a valid photo ID.
- The consent form must be completed using black ink and must be notarized.

Requirements:
1. Proof of U.S. Citizenship (Child Passport)
- Original or certified birth certificate
- An expired previous passport
- Original citizenship certificate
2. Proof of Identification (Parents)
- Parents' State-issued Driver's license
- Parents' Military I.D. bearing photo
3. Passport Photo Requirements (2x2)
- You must submit two 2x2 passport size photos
4. Complete the Online New Passport Application (Form DS11)
- Once you complete the application form you must print, but do not sign the form until requested to do so by an authorized agent
