PASSPORT NAME CHANGE INSTRUCTIONS:
You are eligible to apply for a new passport if one of the following is true :
- I can submit my most recent U.S. passport book and/or card with this application
- I was at least 16 years old when my most recent U.S. passport book and or card was issued.
- I was issued my most recent U.S passport book and/or card less than 15 years ago.
Requirements:
1. Most recent expired passport:
- You must submit your most recent passport
- You must submit your passport card (if applicable)
2. Name change legal document:
- You must submit original or certified copy of your marriage certificate or court decree ordering name change (all documents will be returned)
3. Passport Photo
- You must submit two 2x2 passport size photos
4. Online completed passport renewal application form (Form DS82)
- Once you complete the application form you must print, sign and date it

5. Proof of Departure
- Copy of travel itinerary
- Company letter (for business travel)
6. Letter of Authorization

- Passport Book $209.53
- Passport and Card $239.53
- Make a check or money order payable to U.S. Department of State
8. Passport Expediting Service Fee:
- 5-8 business days processing $699.00
- 8 -16 business days processing $499.00
- 16 - 21 business days processing $399.00
