EXPEDITED PASSPORT RENEWAL INSTRUCTIONS
You are eligible to apply for passport renewal if the following is true:
- You are eligible to apply for passport renewal if you can submit your most recent U.S. passport book and/or card with this application.
- You must have been at least 16 years old when your most recent passport was issued.
- You were issued your most recent U.S. passport book and/or card less than 15 years ago.
Requirements:
1. Most Recent Expired Passport (Book & Card):
- You must submit your most recent passport
- You must submit your passport card (if applicable)
2. Passport Photo Requirements (2x2):
- You must submit two 2x2 passport size photos
3. Complete the Passport Renewal Application (Form DS-82):
- Once you complete the application form you must print, sign and date it

4. Proof of Departure:
- Copy of travel itinerary
- Company letter (for business travel)
