Lost/Stolen U.S. passport instructions and replacement passport processing options.

Requirements:

1. Proof of U.S. Citizenship

Provide proof of U.S. citizenship including an original or certified birth certificate, an expired previous passport, and/or an original citizenship certificate.

2. Proof of Identification

Submit proof of identification such as a state-issued driver's license and/or military I.D. bearing photo.

3. Passport Photo Requirements (2x2)

Complete the online new passport application form (Form DS-11), then print it, but do not sign the form until requested to do so by an authorized agent.

4. Complete the DS-11 Application

What do I need to replace a lost or stolen passport?
DS-11 is the new passport application for first-time U.S. passport applicants, those whose previous passport was issued over 15 years ago, or those whose passport was issued when they were under age 16.
