PASSPORT NAME CHANGE INSTRUCTIONS:
You are eligible to apply for a new passport if one of the following is true :
- You are eligible to apply for a new passport if you can submit your most recent U.S. passport book and/or card, you were at least 16 years old when it was issued, and it was issued less than 15 years ago (as applicable).
- You may qualify if you can submit your most recent U.S. passport book and/or card with this application.
Requirements:
1. Submit Your Most Recent Passport:
- You must submit your most recent passport
- You must submit your passport card (if applicable)
2. Name Change Legal Document Requirement:
- You must submit original or certified copy of your marriage certificate or court decree ordering name change (all documents will be returned)
3. Passport Photo Requirements (2x2)
- You must submit two 2x2 passport size photos
4. Complete Form DS-82
- Once you complete the application form you must print, sign and date it
